weRpatients™: Social & Health Vulnerability Map

Protecting vulnerable populations with geolocated environmental health intelligence

The Social Vulnerability-Health Map is a digital tool based on geolocation and environmental intelligence that identifies areas of health and social risk where the effects of environment-related diseases - such as avian flu, food infections or heat waves - are most likely to occur.

This solution is aimed at public administrations, NGOs, community health teams and insurers who need to prioritise resources, implement data-driven policies and protect the population most at risk from environmental hazards.

What does this product do?

  • Geolocates vulnerability: Maps in real time the territories with the highest levels of social and environmental vulnerability (limited sanitary infrastructure, rural areas close to farms, lack of preventive information).
  • Triggers early warnings: Automatic notifications when outbreaks or emerging risk factors (such as food-borne infections, zoonotic outbreaks or air pollution) are detected.
  • Facilitates public and community intervention: Helps prioritise areas where education campaigns, medical resources or urgent preventive measures are needed.
  • Integrates diverse data: Combines data from public health, climate, air quality, health access, social indicators and epidemiological sources to create a composite risk index.

Available complements

  • API for integration with municipal or regional health systems.
  • Citizen App version for dissemination of localised alerts.
  • Environmental social risk analysis and consultancy service.

Available as an interactive report, API or web platform with technical support.
Contributes to a fairer, predictive and territorially informed public health approach.

Who is it aimed at?

  • City councils and health authorities: to lead actions of containment and health education.
  • NGOs and social organisations: working with people in vulnerable situations (rurality, poverty, digital exclusion).
  • Emergency services: to activate targeted responses according to health or environmental alerts.
  • Insurance companies or mutual insurance companies: who want to adjust policies or prevention services with an equitable approach.

ESG researchers and consultancies: analysing the impact of the environment on health from a population perspective.

Differential value

  • Based on the H4PWeRpatients data engine, validated in real scenarios.
  • Scalable to regional, national or European level.
  • Customisable by risk type: public health, pollution, digital vulnerability, etc.
  • Interoperable with open data systems, local sensors and health platforms (FHIR/HL7).

Key use cases

  • Management of zoonotic or food-borne disease outbreaks (avian influenza, salmonellosis, etc.) with outbreak localisation and social exposure analysis.
  • Preventive planning in environmental health: early deployment of interventions in areas without access to information or medical services.
  • Design of territorial health equity policies: based on dynamic maps that make structural health inequalities visible
  • Integration in municipal digital health platforms: for faster, data-driven decisions.
  • Activation of community networks: through mobile applications that inform affected citizens in real time.
